At Abraham Healthcare, we provide quality in-home care using skilled nurses and aides for efficient service delivery. We provide services ranging from; assistance to individuals after a stay in the hospital, to aging parent who needs help or companionship to remain in their home or as a respite for the family member who cares for an ailing loved one. We also provide private duty nursing for individuals who needs continued care. Our services are flexible for your needs - we offer care for few hours up to 24 hours a day, every day of week on a short or long term basis. 

 

To request for service, a referral must be made to our agency: 

  1. On the receipt of the referral, the Registered Nurse will conduct an onsite initial nursing assessment to establish the patient’s baseline status, identify needs and determine the level of care necessary to meet the patient’s needs.
  2. The Registered Nurse in addition to the primary care physician plan of care, will also develop an aides care plan for the caregiver to follow and will evaluate all aides before assigning them out to ensure that they qualified to perform the needed care and also meets the requirements of the state regulation.
  3. We respond to every request for service within 24 hours of receipt and we follow up monthly with nurse monitoring.

Personal Care:

Personal care service means a service that an individual would normally perform personally, but for which the individual needs physical assistance because of advanced age, infirmity, or physical or mental limitation. They are non-medical services that extend the ability of people to live in their own homes. These services are provided by our experienced caregivers under the supervision of the Registered Nurse. Abraham Healthcare offers the following personal care services: 

  1. Assistance with one or more activities of daily living  
  2. Delegated nursing functions, such as assistance with or administration of medications   
  3. Ambulation, such as assisting consumer to move about independently, transfers consumer from chair to bed, chair to toilet, and from bed to chair. 
  4. Perform the following additional services which are directly related to our consumer’s medical need and are essential to their health and comfort in the home:
    • Light housekeeping and home management, 
    • Plan and prepare meal according to our consumer’s need,  
    • Feed our consumer who is incapable of self-feeding,  
    • Accompanying the consumer to clinics, physician offices, or other trips which are made for the purpose of obtaining medical diagnosis or treatment,  
    • Shopping for groceries or household item required specifically for the consumer’s health and maintenance.
  5. Personal hygiene and grooming, such as assistance with:
    • Bathing of patient, including bed bath or shower,
    • Skin, hair, mouth and dentures care,
    • Dressing and changing briefs, 
    • Toileting, including going to and from and the use of commode

Attendant Care:

Attendant care services means hand-on assistance specific to the functional needs of a consumer with a physical disability and includes assistance with activities of daily living and instrumental activities of daily living. Attendant care is a living at home waiver program and services includes assistance with:

  1. Bathing and personal hygiene routines,
  2. Toileting, including bladder and bowel requirements, bed pan routines, incontinence care, and movement to and from the bathroom,
  3. Mobility, including transferring from a bed, chair, or other structure and moving about indoors or outdoors,
  4. Moving, turning, and positioning the body while in bed or in a wheelchair,
  5. Eating, nutritional planning, and preparing meals including prescribed special diets,
  6. Dressing and changing clothes,
  7. Medications or other remedies that are ordinarily self-administered, when ordered for the consumer by a physician
  8. Instrumental activities of daily living which includes:   

    • Preparing a light meal,
    • Perfoming light chores,
    • Shopping for groceries,
    • Traveling beyong a walking distance,
    • Reading of specific items, and
    • Planning and making decisions

Respite Care:

Respite care provides short-term supportive care to an individual when the primary caregiver is unavailable due to a planned temporary absence, unexpected illness, emergencies or need some rest and relief. Abraham Healthcare can provide capable caregivers that can assist for as little as a few hours to long weeks of vacation. At Abraham Healthcare Services, respite care are:

  1. Provided to consumers who are, due to physical or cognitive impairments, are unable to care for themselves;
  2. Furnished on a short-term basis because of the absence or need for relief of a family member or individual normally providing the the care;
  3. Provide a period of rest and renewal for the family member or other caregiver;
  4. Provided at planned intervals, in a time of crisis, as needed;
  5. Provided in the consumer's home or place of residence, or an assisted living facility.

Services offered includes:

  1. Supervising a cognitively impaired consumer to asssure their health and safety; and
  2. Assisting a functionally impaired consumer to perform activities of the daily living.

Live-in care:

In some cases, your loved ones need constant care and supervision. We encourage you to consider having a live-in caregiver over putting your loved one in a nursing home or Assisted Living facility. Live-in care is when a caregiver resides with the client to service all of the client’s needs on a daily basis. Our caregivers  adjust to the clients’ need and schedule to provide the highest level of care needed. It is highly speculated that loved ones cared for at the home live much longer and happier than those cared for in nursing facilities. Our live-in caregivers become part of the family and render support throughout the course of each day. Live-in care services offered includes: 

  1. Personal care, to include: bathing, showering, dressing, toileting etc.
  2. Medication reminder/assistance
  3. Assistance with mobility
  4. Shopping and errands
  5. Preparing and cooking of meals & snacks
  6. Laundry/linen change

Companion Care:

A companion care service consists of non-medical care, supervision, and socialization activities provided on a one-on-one basis to an adult or children. All that is needed sometimes is a care and company. In most situations, having another individual in the house with your loved ones gives you the added assurance you need in case of an emergency. All of our companions are experienced, trained, CPR and First Aid certified. Services Offered includes:

  1. Companionship and safety supervision; includes accompanying consumer on walks, accompany to restaurants, stand-by bathing assistance,
  2. Conversation- cognitive stimulation, includes playing games or cards, read aloud from books/periodicals, help with hobbies/crafts with the consumer, 
  3. Medication reminders,
  4. Meal preparation, includes planning of meals, monitoring diet, and serving meals to consumer,
  5. Shopping and errands; includes picking up meals,  pick up/drop off dry cleaning, and bringing in mail,
  6. Light housekeeping; includes making of bed,  mopping and sweeping, dusting, vacuuming floors, washing and arranging dishes, laundry and linens changes, kitchen and bathroom cleanups, garbage disposal. 
  7. Assist with grooming & dressing.

Skilled Nursing:

We provide skilled nursing services, within defined scope of practice, to consumers based on the initial nursing assessment and the care plan develop by the registered nurse. Skilled Nursing Care is provided for patients who require intensive medical support following an illness or accident. Abraham Healthcare offers the following skilled nursing services:

Private Duty Nursing:

Private Duty Nursing  is the provision of skilled nursing care in the home by a Registered Nurse or a Licensed Practical Nurse. The purpose of private duty nursing is to assess, monitor and provide skilled nursing care in the home on an hourly basis; to assist in the transition of care from a more acute setting to home; and to teach competent caregivers the assumption of this care when the condition of the member is stabilized.

Abraham Healthcare responds to all referrals within 24 hours of receipt and has a registered nurse on call.

How do you pay?:

We accept individuals with: 

  • Medicaid [Waiver for Older Adult & Living at Home Waiver],
  • Some Private Insurance
  • Private Pay and,
  • we offer sliding scale fee for Individuals with no medical insurance.
